Along with vaccination records, a signed copy of the policy agreement must be given by mail prior to class time. Will the main registrant please initial each space as well as have each attendee and self sign at the bottom to acknowledge accepting these classroom policies.
Policies:
Dogs must be current on vaccinations, including Rabies, Bordatella, and DHLPP, and good health certification, for all sessions. Proof is required in addition to this policy agreement and payment. _____
All dogs must be on leash or on their 'tie on' at all times unless otherwise directed. Slip chains (choke), pinch collars and halti/head harnesses are not allowed. Body harnesses, or standard collar and leashes are accepted. _____
All dogs should be in good health for the classes. If you dog is acting ill, please call us *after you call your veterinarian*. This goes especially for coughing, wheezing, lethargy, vomiting or diarrhea. Often, we will credit you the course and let you pick back up again for the next round, or give you private instruction to supplement what you missed. _____
Contact between dogs, even greetings, we ask to be structured - by requesting with the Owner & Trainer first, and not at all on the first session. This is a safety for all precaution. (We've never had a classroom accident, and don't plan to). Please allow for a distance of 5 feet including when leashes are fully extended between other dogs and owners, unless directed. Even people not accompanied by other dogs must still ask to approach and pet other student dogs. All attendees have been instructed and agree to this ettiquette. _____
Children age 9 an under are asked to be accompanied by a non-participating adult who assures their appropriate behavior there. Older children may sit and quietly watch the classes, and may even earn participation if they are very focused themselves (it is a case by case situation). By signing below, the parent has explained to them about the classroom rules and responsibilities. _____
Female dogs in heat are unfortunately unable to attend. If your dog enters heat during a class session, you will be offered to finish the course on the next round. Sorry there are no refunds in this case. _____
Pottying accidents happen.. please remain near the spot and call for clean up, not allowing your dog to track in it as well. I, or my assistant, will attend to you quickly. ______
Animals, guests or handlers disruptive to the class room, or highly aggressive dogs, may be asked by the trainer to be removed from the class room. If you have concerns about your pets behavior, please notify the trainer prior to enrollment. _____
Refunds *or* credits are offered (your choice) if a course is cancelled. Two weeks prior to classes beginning, there are no refunds for pet owner cancellations, though credits are offered on a case by case basis regarding transferring to another course, or health related issues. Walk ins for training incur a 10% fee, if space is available for them, and they have the necessary documents. However, any person wishing to visit one standard class hour and observe is welcome to, but must do so without their pet and still sign a policy agreement. _____
Payment is due for reservation, whether paid by credit card, mailed or in person prior to classes beginning. Reservations are not held without payment, policy agreement and vet records combined - there is no exception. _____
Returned (bounced) Checks have an addition fee of 55.00, to cover my banks fees as well as time collecting cash payment. _____
Certificates of Completion are offered with each course. Missed classes prevent a completion certificate unless all pertinent skills (perk skills not necessary) can be shown on the last day. Pets and human both get treats upon graduation as well. _____
Seating is limited, so please announce how many is coming ahead of time so we can be sure to accommodate you comfortably. _____
By signing all clients agree to maintain complete control of their dogs, and assumes full responsibility there of. Each attendee and visitor releases trainer and property from all claims, recognizes there is an element of risk in working with animals and their owners, and will honor their responsibility of their pet and themselves consistently. Each student accepts the entire responsibility of their guests as well. _____
Each guest who is not accompanying a client must stay seated, not bring their own pet, and not disrupt the class please. _____
